Official Transcripts

Official LSU Health Sciences Center at New Orleans transcripts of an academic record may be ordered online through Parchment® at http://www.iwantmytranscript.com.   The cost of an official transcript is $7.25 per recipient processing fee.

Individuals who are unable to use the online service may submit a signed written request along with a check or money order in the amount of $7.25 per recipient processing fee.  Express delivery options are available.

All transcript requests must include the following information:

• Name (If the current name is different from the name under which you were enrolled, the former name must also be referenced.)
• Social security number or ID number
• Mailing Address
• Years of attendance
• School attended (Allied Health Professions, Dentistry, Graduate Studies, Medicine, Nursing, or Public Health)
• Signature

Express Delivery Options

You can request express delivery of your transcript if you choose this delivery method in your online order and include payment to cover the additional postage fees.  No PO Box addresses for Domestic Federal Express options.

Next Day Delivery Through FedEx:

•   Within the Continental United States - $25.00
•   Alaska and Hawaii - $40.00
•   International deliveries - $45.00

Fees are in addition to the transcript fee of $7.25 each.

All fees are subject to change. 

The Family Education Rights and Privacy Act requires the school to have a signed and dated written release before issuing transcripts.

The transcript will usually be mailed from the Office of the Registrar within one business day of receipt of an online request, and within five business days for written requests, except during periods of high demand.

Unofficial Transcripts

Currently enrolled student may obtain unofficial transcripts, free of charge, through the student self-service portal at http://academicselfservice.lsuhsc.edu.
